Patient is a 52 year old who underwent skin sparing mastectomy with immediate expander which was later changed out to an implant (750cc anatomic). She was seen with asymmetry, capsular contracture and no distinct fold. She is 1 1/2 years post removal of implant and capsule, reduction of implant to 450cc round and advancement of the upper abdominal tissues to provide more skin for the breast, create a fold and allow for ptosis (Ryan advancement flap).
